the first poster is correct

It is called Japanese straightening...get a phone book, Yellow Pages, and call various salons and ask if they do it

It is very expensive and the roots will grow out curly but as the first poster said, your hair won't be the same...you might have to cut it all off and start over...the roots will be kind of curly and the rest of the hair will be permanently straight

It is possible. It costs about $300. It last about 3 months to a year. It depends on how fast your hair grows. Just as if you dyed your hair the color will grow out when you permantely straighten your hair it too will grow out.

What I do, is use a dark and lovely hair relaxer, which loosens the curls, and then I flat iron it straight. You're supposed to use the relaxer every 6 weeks, but I usually get away with 2 months. Depending on what strength you get in the relaxer, flat ironing your hair can be fairly easy.
